Then the individual youth and children's worker shoudl complete the form below using BLOCK CAPITAL letters.

The information the worker provides is manually entered on to a Disclosure Application which they will be required to check and sign at the identification verification stage of the process, for which we also require at least one photographic ID and two proofs of address, such as a utility bill and a bank statement.
